On your first visit, please complete necessary paperwork prior to your appointment time. You can download the forms from this website and fill them out in advance.
Out of respect for my time and yours, I request a 24 hour notice for all cancellations. Although I understand unforeseen events occasionally arise, I reserve the right to charge for cancellations.
